Admissions
The admission process helps us learn information that is vital
to the care you will receive while hospitalized as well as the
information needed by a variety of hospital services. Law mandates
some questions and we appreciate your assistance in completing
this process. At the time of your admission, you will be asked
to present insurance information for proper handling of your claim
or you will be encouraged to make payment arrangements. On the
day of admission, please bring the following items to the admitting
office:

Emergency notification
of someone outside your home, including name, address,
telephone number and relationship
•
Any required insurance forms
•
Advanced directive
•
Social security card of the patient
compensation claim and the insured
•
Medicare or Medicaid card
•
Insurance identification cards
•
Driver's license and/or photo identification
card of the patient and the insured
•
Incident report if filing a workers
compensation claim
